ژن FUT3 ، بیان آنتیژنهای گروه خونی لوییس از جمله Lea و Leb را تنظیم می کند. افرادی که آنتیژنهای گروه خونی لوییس را نمی سازند، هاپلوتایپ هایی با پلیمورفیسمهای تک نوکلیوتیدی غیر فعال کننده آنزیم دارند که فراوانی آن ها در بین نژادهای مختلف متفاوت است. در مطالعه حاضر، فراوانی این پلیمورفیسمها در اهداکنندگان ایرانی مورد بررسی قرار گرفت.
مواد و روش هادر این مطالعه مقطعی- توصیفی، 100 نفر از اهداکنندگان مراجعهکننده به پایگاه انتقال خون تهران در سال 1396 مورد بررسی قرار گرفتند. از PCR و DNA sequencing برای تعیین پلیمورفیسمهای تک نوکلیوتیدی ژن لوییس استفاده شد. هاپلوتایپها نیز توسط نرم افزار SNP Analyzer مشخص شدند.
یافته هادر مجموع 15 پلیمورفیسم شناسایی شد که از این تعداد 10 مورد مرتبط با موتاسیونهای missense بودند. فراوانی مهمترین پلیمورفیسمهای شناسایی شده عبارتند از: 202T>C 39%، 314C>T 37%، 59T>G 20% ، 508G>A 9% و 47G>C 8% . هم چنین 13 هاپلوتایپ نیز شناسایی شد که فراوانی مهمترین هاپلوتیپها عبارتند از: Le 2/66%، le202,314 5/17%، le47,202,314 3/4%، le59 1/4% و le59,508 4/3% .
نتیجه گیریبر اساس این پژوهش، پلیمورفیسمهای 202T>C، 59T>G و 508G>A میتوانند به عنوان SNP های اصلی برای شناسایی آللهای لوییس منفی در تعیین ژنوتیپ گروههای خونی لوییس و مطالعه های مرتبط با بیماریها مفید واقع شوند.

تجویز غیر ضروری خون، علاوه بر افزایش هزینه های درمانی می تواند باعث عوارض متعددی در بیماران شود. هدف این مطالعه، بررسی الگوی مصرف خون و ارتباط آن با علل مرگ و میر در بخش ICU مرکز آموزشی و درمانی بعثت همدان در سال 98-97 بود.
مواد و روش هااین مطالعه مقطعی و توصیفی- تحلیلی در سال 1398 انجام شد. جهت جمع آوری داده های دموگرافیک و بالینی بیماران واجد شرایط، از یک چک لیست استفاده شد. نهایتا داده ها با آزمون های آماری غیر پارامتریک و همبستگی اسپیرمن، کای دو و توسط نرم افزار آماری SPSS نسخه 22 مورد تجزیه و تحلیل قرار گرفت.
یافته هامجموعا 227 بیمار شامل 130 فرد مذکر (3/57%) و 97 فرد مونث (7/42%) با میانگین سنی 83/12 ± 02/39 سال مورد بررسی قرار گرفتند. میانگین واحد خون دریافتی توسط هر بیمار 02/1 ± 68/1 و میانگین مدت بستری بیماران 03/6 ± 43/8 روز بود. 4/19% بیماران فوت نموده بودند و شایع ترین عامل مرگ (4/8%)، بیماری/آسیب سیستم مغزی و عصبی بود. بیشترین فرآورده خونی مورد استفاده، گلبول قرمز متراکم (70%) بود. رایج ترین اندیکاسیون تجویز خون، جراحی (3/75%) و خونریزی (5/7%) بود. بین تعداد واحد خونی با پیامد نهایی بیماری، ارتباط مستقیم و معنادار وجود داشت (001/0 <P).
نتیجه گیریاز آن جایی که بیشترین نوع فرآورده خونی استفاده شده، گلبول قرمز متراکم بود و بین تعداد و نوع واحد خونی تجویز شده با مدت بستری و بروز مرگ ارتباط مستقیم وجود داشت، توصیه می شود از تجویز بی رویه و غیر ضروری گلبول قرمز فشرده خودداری گردد.

در حیطه مراقبتی کودکان، شرایط بالینی نیازمند به تزریق خون بسیار اورژانسی بوده و تزریق به موقع در بیشترمواقع منجر به بهبود پیامدهای بالینی می گردد، لذا جهت اطمینان از کفایت شایستگی های بالینی دانشجویان پرستاری در این حوزه، ارزیابی میزان دانش و تصمیم گیری بالینی دانشجویان انجام گردید.
مواد و روش هااین مطالعه توصیفی- مقطعی در سال 1398 بر روی 110 دانشجوی پرستاری دانشگاه علوم پزشکی گیلان، به صورت در دسترس انجام گردید. داده ها از طریق پرسشنامه طراحی شده توسط محقق در سه بعد مشخصات فردی، دانش و تصمیم گیری بالینی دانشجویان در خصوص کاربرد طب انتقال خون در پرستاری کودکان پس از تایید روایی و پایایی جمع آوری گردید. برای تحلیل داده ها از آمارهای توصیفی و استنباطی پارامتریک و غیر پارامتریک با نرم افزار آماری 16 SPSS استفاده شد (05/0 <P).
یافته هامیانگین به درصد نمرات دانش دانشجویان 06/23 ± 57/50 و تصمیم گیری بالینی دانشجویان 18/20 ± 33/31 بود. ارتباط مثبت مستقیم معناداری بین نمرات دانش و نمره تصمیم گیری بالینی وجود داشت (49/0 r = ، 001/0 <P). بین تجربه کار دانشجویی، سابقه دریافت خون در خویشاوندان و تجربه مراقبت از بیمار دریافت کننده خون در کارآموزی با نمرات دانش و تصمیم گیری بالینی ارتباط معناداری وجود داشت.
نتیجه گیریبا توجه به سطح متوسط دانش و سطح پایین تصمیم گیری بالینی دانشجویان، جهت ارتقای ایمنی بیماران، بهتر است دست اندرکاران آموزش قبل از فارغ التحصیلی دانشجویان با برگزاری دوره های تکمیلی و کاربرد روش های آموزشی مبتنی بر شایستگی بالینی، از فرصت های یادگیری بالینی در حوزه طب انتقال خون استفاده نمایند.

ویروس لنفوتروپیک سلول T انسانی نوعI ، با دو بیماری در انسان یعنی لوکمی/لنفوم T بزرگسالان و پاراپارسیس اسپاستیک گرمسیری مرتبط است. حدود 5 تا 10 میلیون نفر با HTLV-1 در سراسر جهان آلوده شده اند. به منظور افزایش سلامت خون در گیرندگان و جلوگیری از نتایج کاذب در داوطلبان اهدای خون، نیاز است روش های شناسایی عوامل عفونی، از حساسیت و اختصاصیت بالا برخوردار باشند. هدف از این مطالعه؛ مقایسه آزمایش های الکتروکمی لومینسانس، وسترن بلاتینگ و PCR ، برای تایید ویروس HTLV-1 بود.
مواد و روش هااین مطالعه تجربی در سال 1397 بر روی تعداد 66 نمونه(62 مرد و 4 زن) که با کیت الایزا دارای آنتی بادی علیه ویروس HTLV-1بودند و با روش های الکتروکمی لومینسانس و وسترن بلات آزمایش شدند، انجام شد. هم چنین جهت تشخیص ویروس با روش PCR ، اهداکنندگان خون مجدد فراخوان شدند و نمونه جدید از آنان اخذ شد. پس از استخراج DNA ، آزمایش Nested PCR با آغازگرهای هر دو ناحیه TAX و LTR انجام شد.
یافته هادر این مطالعه تعداد 8 (1/12%) نمونه از 66 نمونه که با آزمایش الایزا، مجددا واکنش نشان داده بودند، انتخاب شدند. تعداد 4 (6%) نمونه با روش الکتروکمی لومینسانس، حاوی آنتی بادی علیه HTLV-1 بودند. در بررسی با روش های وسترن بلات و Nested PCR نیز، همان 4 (6%) نمونه مجدد مثبت شدند.
نتیجه گیریاستفاده از آزمایش های الکتروکمی لومینسانس، وسترن بلات و روش PCRکه در این مطالعه نتایج مشابهی داشتند، برای تایید ویروس HTLV-1 مناسب می باشند.

با توجه به تنوع بالا در میزان مصرف خون در مراکز درمانی مختلف، مطالعه عملکردهای حاکم بر انتقال خون در مراکز بیمارستانی می تواند بر سیاست گذاری و برنامه ریزی بیمارستان ها درخصوص مدیریت صحیح مصرف فرآورده های خون موثر باشد. مطالعه حاضر به منظور بررسی عملکرد های حاکم بر انتقال خون در بیمارستان قلب شهید رجایی تهران انجام شد.
مواد و روش هادر این مطالعه مقطعی و آینده نگر، 399 بیمار بزرگسال تحت عمل جراحی قلب در مرکز قلب شهید رجایی تهران از بهمن ماه 1396 تا تیر ماه 1397 مورد مطالعه قرار گرفتند. اطلاعات مربوط به میزان خون رزرو شده، مصرف فرآورده های خونی و فراوانی بیماران دارای انتقال خون مثبت و با توجه به نوع عمل، مقایسه گردید. جهت تحلیل داده ها از آزمون های آماری کای اسکویر، آنالیز واریانس یک طرفه و نرم افزار SPSS 23 IBM استفاده شد.
یافته هافراوانی انتقال خون در جمعیت مورد مطالعه برابر 71% (282) بود. 65% (259) RBC ، 21% (83) FFP و 18% بیمار (71) پلاکت دریافت کرده اند. از 749 واحد RBC رزرو شده، 256 واحد(34%) برای بیماران استفاده شد. 25% از بیماران قبل از عمل کم خون بودند و میانگین هموگلوبین قبل از تزریق g/dL RBC 3/1 ± 3/10 بود. اختلاف معناداری بر اساس نوع جراحی در تعداد بیماران دارای انتقال خون و واحدهای خون مصرف شده وجود نداشت.
نتیجه گیریبا توجه به بالا بودن میزان انتقال خون در این مرکز، پیروی از دستورالعمل های انتقال خون سختگیرانه در رابطه با آستانه هموگلوبین و تهیه و اجرای دستورالعمل اصلاح کم خونی قبل از عمل و MSBOS توصیه می شود.

برای ترغیب زنان به اهدای خون، باید باورها و عواملی که مانع اهدای خون در آن ها می شود شناسایی و بررسی گردد. هدف از این مطالعه؛ بررسی مشارکت زنان در اهدای خون با طرح پرسشنامه می باشد که نتایج حاصل می تواند جهت آموزش بهتر بانوان و رفع موانع بر سر راه اهدای خون آن ها موثر واقع شود.
مواد و روش هادر یک مطالعه مقطعی، پرسشنامه توسط 250 نفر از شرکت کنندگان تکمیل و اطلاعات وارد نرم افزار 26 SPSS شد. با استفاده از آزمون های همبستگی پیرسون، تحلیل واریانس یک طرفه و رگرسیون، تجزیه و تحلیل آماری انجام شد.
یافته ها:
6/93% افراد، مشارکت در نجات جان بیماران را عامل موثر در امر اهدای خون و 2/81% افراد، ابتلا به کم خونی را به عنوان موانع عدم مشارکت عنوان نمودند. متغیر آگاهی از شرایط اهدای خون با عوامل موثر در اهدای خون رابطه معکوس و معناداری داشت ولی امتیاز موانع با مشارکت رابطه معناداری نداشت (003/0=p). بر اساس نتایج آزمون T مستقل، ارتباط وضعیت تاهل با امتیاز آگاهی، امتیاز موانع عدم مشارکت در اهدای خون و امتیاز عوامل موثر در مشارکت در اهدای خون با تاهل ارتباط معناداری نداشتند. نتایج تحلیل واریانس نشان داد سطح تحصیلات با امتیاز آگاهی ارتباط معناداری نداشت اما با امتیاز موانع عدم اهدای خون رابطه معکوس و معناداری نشان داد (017/0=p).
نتیجه گیریبا توجه به موانع اعلام شده و روابط حاصل در یافته های این مطالعه برای مشارکت در امر اهدای خون، فرهنگ سازی صحیح پیرامون فرآیند اهدای خون می تواند راه کار مناسبی جهت ترغیب بیشتر و بهتر بانوان باشد.

سلول های بنیادی خونساز خون بند ناف،دارای توانایی های بالقوه زیادی جهت درمان بیماری های هماتولوژیکی و غیر هماتولوژیکی می باشند. آگاهی از بیولوژی ، خود نوسازی ، لانه گزینی ، تکثیر ، ذخیره سازی و پیوند سلول های بنیادی خونساز خون بند ناف، می تواند منجر به استفاده بهینه از این سلول ها گردد.
مواد و روش هادر این مطالعه مروری، جهت بررسی پیشرفت ها و چالش های موجود در بانک های خون سلول های بنیادی بند ناف، روش های تکثیر، ذخیره سازی، لانه گزینی و پیوند با استفاده از کلید واژه های خون بند ناف، سلول های بنیادی خونساز و بانک سلول های بنیادی به جستجوی منابع در پایگاه اطلاعاتیMed Pubدر بازه زمانی 2000 تا 2020 پرداخته شد.
یافته هابا گذشت زمان، پیشرفت های زیادی در زمینه بیولوژی، تکثیر، ذخیره سازی و پیوند سلول های خونساز بند ناف توسط محققان در سراسر دنیا ایجاد شده است و به موازات آن بانک های خون خصوصی و دولتی در سراسر دنیا گسترش و توسعه پیدا کرده اند. با وجود این پیشرفت ها، هم چنان چالش هایی جهت استفاده بهینه از این سلول ها وجود دارد.
نتیجه گیریافزایش آگاهی ما نسبت به دستاورد ها و کاستی های موجود در زمینه سلول های بنیادی خونساز خون بند ناف می تواند منجر به شکل گیری راه کار های جدید و انجام مطالعه های بیشتر جهت استفاده بهینه از سلول های بنیادی خونساز خون بند ناف گردد.

تزریق پلاکت، یکی از درمان های اصلی بیماران ترومبوسیتوپنیک، جهت کاهش شدت و فراوانی عواقب ناشی از خونریزی می باشد. عدم افزایش متناسب پلاکت ها متعاقب تزریق آن ها تحت عنوان مقاومت پلاکتی شناخته می شود. هدف از این مطالعه، بررسی مکانیسم های دخیل در بروز و شکل گیری مقاومت پلاکتی بود.
مواد و روش هااین مقاله به مرور فاکتورهای موثر در افراد دریافت کننده و اهداکننده پلاکت و برخی از خصوصیات خود محصول پلاکتی در بروز مقاومت پلاکتی پرداخته است که از طریق بانک های اطلاعاتی Science Direct ، PubMed ،Medline ،SID ،Scopus و Magiran و جستجوی کلید واژه های مقاومت پلاکتی، آلوآنتی بادی، HLA و تزریق پلاکت در بین حدود 130 مقاله مرتبط انجام شده و نهایتا 97 مقاله برای نوشتن استفاده گردید.
یافته هامقاومت پلاکتی به دو گروه مقاومت ایمونولوژیک و غیر ایمونولوژیک قابل تقسیم می باشد. نوع ایمونولوژیک عمدتا با ایجاد آلوایمونیزاسیون ناشی از تماس با آنتی ژن های لکوسیت انسانی -HLA و آنتی ژن های پلاکت انسانی HPA- ایجاد می گردد. در موارد غیر ایمونولوژیک که حدود80% علل بروز مقاومت پلاکتی به شمار می آیند، عواملی هم چون تب، بزرگی طحال و مصرف برخی داروها منجر به بروز مقاومت پلاکتی می گردند.
نتیجه گیریشناسایی عوامل و مکانیسم های دخیل در ایجاد مقاومت پلاکتی اعم از عوامل ایمونولوژیک و غیر ایمونولوژیک، نقش مهمی در پیشگیری، مدیریت بیماری و جلوگیری از گسترش و تکرار آن بر عهده خواهد داشت.

The FUT3 gene regulates the expression of Lewis blood group antigens mainly Lea and Leb. The Lewis negative phenotype, is the result of an inactivated FUT3 enzyme that lacks glycosidase activity. Several single nucleotide polymorphisms (SNPs) may cause enzyme inactivation with different racial distribution. This study aimed to determine the frequency of these SNPs in Iranian blood donors.
Materials and MethodsIn this cross sectional descriptive study, one hundred blood donors from Tehran Blood Transfusion Center were analyzed. Genomic DNA was extracted and the coding sequence of FUT3 gene was amplified with specific primers. PCR products were directly sequenced. Haplotypes were inferred by SNP Analyzer software.
ResultsA total of 15 SNPs including 10 nonsense SNPs were recognized. The frequency of most common SNPs was detected as 39%(202T>C), 37%(314C>T), 20%(59T>G), 9%(508G>A), and 8%(47G>C). Haplotype analysis revealed 13 haplotypes. The frequency of major haplotypes was detected as 66.2%(Le), 17.5%(le202,314), 4.3 %(le47,202,314), 4.1 %(le59), and 3.4%(le59,508).
Conclusions :
Our findings indicate that 202T > C, 59T > G and 508G > A are useful as major SNPs for detection of Lewis-negative alleles in genotyping of Lewis blood groups; they can also play an effective role in large-scale studies associated with diseases in our population.

Administering unnecessary blood can cause many complications in patients and add to medical costs. This study aimed to evaluate blood consumption pattern and its relation with causes of mortality in Geneal ICU Ward of Besat Educational Medical Center of Hamadan in 2018-2019.
Materials and MethodsThis cross-sectional study was performed retrospectively in 2018-2019 on all eligible patients. A checklist was developed to collect demographic and clinical data. Finally, the data were analyzed by Kruskal-Wallis Test, Spearmanchr('39')s Correlation Coefficient, Chi-square Test, Mann-Whitney U by SPSS version 22.
Results227 patients including 130 (57.3%) males and 97 females (42.7%) with the mean age of 39.02 ± 12.83 years were studied. The mean blood unit administered to each patient was 1.68 ± 1.02 and the mean hospital stay period was 8.43 ± 6.03 days. 19.4% of the patients died and the most common cause of death (8.4%) was attributed to brain and nervous system diseases/ damages. The most common type of blood products used was packed red blood cells (70%). The most common indication for blood prescription was surgery (75.3%) followed by bleeding (7.5%). There was a direct and significant relationship between the numberblood units and final outcome of the disease (p < 0.001).
ConclusionsSince the most common type of administered blood products was packed RBCs and there was a direct relationship between the number of prescribed blood units and the incidence of death, it is recommended to avoid unnecessary administration of packed RBCs.

Lack of knowledge in health care providers may lead to an increased risk and associated complications of blood transfusion. In pediatric nursing care, clinical conditions requiring transfusion are highly urgent. Therefore, assessment of knowledge and clinical decision-making was performed to ensure clinical competency of nursing students.
Materials and MethodsThis cross-sectional study was performed on 110 nursing students in 2020 by convenience sampling. Data collection was done through a researcher-made questionnaire that was made valid and reliable. The questionnaire included questions about personal characteristics, and those addressing knowledge level and clinical decision-making competence of students. The data was analyzed by SPSS 16 software (p<0.05).
ResultsThe majority of students (97.3%) were under 25 years old, and 55.5% were women. The mean percentage of studentschr('39') total scores in knowledge areas regarding the use of blood transfusion in pediatric nursing was 50.57 ± 23.06. Moreover, the mean score of clinical decision making competence was 31.33 ± 20.18. There was a significant direct relationship between studentschr('39') knowledge scores and clinical decision-making scores (r = 0.49, p < 0.001). There was a significant relationship between students’ knowledge and clinical decision-making and some factors including studentchr('39')s occupation, history of transfusion on a family member, and experience of caring for a blood recipient during the apprenticeship periods.
Conclusions :
As nursing students lack a sufficient level of knowledge and ability of clinical decision-making, further training courses emphasizing clinical-competency-based educational practices in the field of blood transfusion medicine to improve the patient health are required.

Human T-cell lymphotropic virus type I (HTLV-I) has been associated with two diseases in humans such as adult T-cell leukemia/lymphoma (ATLL) and HTLV-associated myelopathy/tropical spastic Para paresis (HAM/TSP). About 5 to 10 million people are infected with HTLV-1 worldwide. In order to improve the blood safety and to prevent false positive results in blood donors, methods for identifying infectious agents need to be highly sensitive and specific. The purpose of this study was to compare the electrochemical luminescence, western blotting, and PCR assays to confirm HTLV-1 virus.
Materials and MethodsThis exprimental study was carried out in 2018 on 66 samples (62 males and 4 females) which had antibody against HTLV-1 virus. All retested samples with the same ELISA kit were examined by electrochemical luminescence and western blotting. For virus detection by PCR method, blood donors were recalled for new samples. After DNA extraction, nested PCR was performed with primers in both TAX and LTR regions.
ResultsIn this study 8 (%12/1) samples from 66 samples were reacted with ELISA test. Also 4 (%6) samples had antibody against HTLV-1 by electrochemiluminescence test. Finally, four samples were confirmed by Western blotting and Nested PCR.
Conclusions :
The use of the electrochemical luminescence, western blotting and PCR assays, which had similar results in this study, is suitable for detection and confirmation of HTLV-1 virus.

Considering with the variability in the rate of blood and blood consumption in different medical centers, study of the transfusion practices in hospitals can affect the policies and planning related to the transfusion management. The aim of this study was to evaluate the transfusion practices in Rajaie cardiovascular hospital.
Materials and MethodsIn this cross sectional study, we collected data from 399 adult patients underwent cardiac surgery at Ri cardiovascular medical center from February to July 2018 .The number of reserved and transfused units of blood components, frequency of transfused patients, and the frequency of blood components consumption were recorded. The frequency of transfused patients and number of blood components consumed were compared considering with the type of surgery. The data were analyzed by one way ANOVA and chi-square tests.
Results71% (282) of patients received at least one unit of blood product. 65% (259) of patients received RBC, 21% (83) FFP and 18% (71) received platelet. Of the 749 reserved RBC units, 256 units (34%) were transfused. 25% of patients had preoperative anemia. The mean trigger of hemoglobin for transfusion was 10.3 ± 3.1 g/dL. Comparison of the frequency of transfused patients as well as mean number of blood components did not show significant difference considering with the type of surgery.
ConclusionsConsidering with the high rate of transfusion in this center, following a restrictive threshold of hemoglobin according to the blood transfusion guidelines as well as implementation of a preoperative anemia management guideline and MSBOS is recommended.

To encourage women to embark on blood donation, the opinions and factors that impede blood donation should be identified. The purpose of this study was to investigate factors affecting participation of women in blood donation.
Materials and MethodsIn this cross-sectional study, the questionnaires were completed by 250 female staff of Tabriz University of Medical Sciences. Data were analyzed using Pearson correlation, one-way ANOVA and regression analysis.
Results93.6% of people claimed blood donation is a humanitarian act, and 81.2% claimed "anemia" is barrier of blood donation. Regression analysis showed that there was a negative and significant relationship between knowledge and motivating factors that have impact on blood donation (p= 0.003), but barriers score did not show a significant correlation with women participation (p= 0.515). Based on the results of independent T-test, the relationship of marital status was not significant as compared with the knowledge score (p = 0.834), with the barriers score (p= 0.434), and with that of motivating factors (p = 0.615). Education level has not significantly correlated with the knowledge score (p = 0.510) but there was a significant negative correlation with barriers variable in blood donation (p = 0.17).
Conclusions :
According to the results, most of the participants in this study considered fear of anemia and fear of test results as barrier of blood donation. Therefore, establishing basic education and training programs to eliminate the wrong beliefs about blood donation would help encourage women as healthy sources of blood donors.

Umbilical cord blood hematopoietic stem cells (UCB-HSCs) have high potential capabilities in the treatment of hematological and non-hematological disorders. Awareness of biology, self-renewal, homing, expansion, storage, and transplantation can lead to optimal use of these cells.
Materials and MethodsIn this Review article in order to investigate the advances and challenges in cord blood banks, the expansion, storage, homing and transplantation of umbilical cord blood stem cells, we used key words like umbilical cord blood, hematopoietic stem cells and stem cell banks for searching published articles in the PubMed database during 2000 to 2020.
ResultsOver time, many advances in biology, expansion, storage, and transplantation of cord blood cells have been made by researchers around the world with a growth in the number of private and public cord blood banks in parallel. Despite these advances, there are still challenges to the optimal use of these cells.
Conclusions :
Increasing our awareness about the achievements and shortcomings in the area of UCB-HSCs, can lead to the formation of new strategies and further studies for the optimal use of these cells.

Transfusion of platelet has become one of the main treatments for thrombocytopenic patients to reduce the severity and frequency of bleeding consequences. Failure to attain the desired level of platelets in a patient following platelet transfusion is defined as platelet refractoriness. The purpose of present paper is to review and study the mechanisms involved in the formation of platelet refractory.
Materials and MethodsThis article reviews the effective factors present in platelet recipients and donors, and considers the characteristics of platelet products that may have impacts on platelet refractoriness. The search is conducted through scientific databases including Science Direct, PubMed, Medline, SID, Scopus, and Magiran by the keywords “Platelet Refractory”, “Alloantibody”, “HLA”, “Platelet Transfusion” and their Persian equivalents. Finally out of about 130 related articles, 97 articles were used for the review.
ResultsGenerally, platelet refractoriness occurs in two ways including immunologic and non-immunologic. The immunologic way is due to alloimmunization against alloantigens as a result of contact with human leukocyte antigens (HLA), Human Platelet Antigens (HPA). Non immunologic conditions such as fever, spleen enlargement and use of certain medications are responsible to 80% of platelet refractory incidence.
Conclusions :
Identification of different features and mechanisms in platelet refractoriness including immunologic and non-immunologic factors have an important role in control, prevention and management of the disease.
